UK Visa rules have changed, what this means for your treatment journey

Yesterday, the United Kingdom introduced one of the most biggest travel changes to affect St Lucian nationals in recent memory. From 5 March 2026 at 3pm GMT, St Lucian nationals must now have a visa to visit the United Kingdom, including those who are only passing through on the way to another destination.

For Orbital Well clients travelling to India or other international destinations for medical treatment, many of whom use the UK as a transit hub or stopover point, this change has immediate and practical consequences. Here is what you need to know.

What’s changed?

Until 5 March 2026, St Lucian nationals could travel to the UK using an Electronic Travel Authorisation (ETA), a straightforward, low-cost online application. That option is now closed. St Lucian nationals now need a visit visa before travelling to the UK, and they will also need a Direct Airside Transit Visa when transiting through the UK on the way to a third destination. The ETA scheme no longer applies.

A six-week transition period is in place, running until 3pm BST on 16 April 2026. During this period, St Lucian nationals who already hold a confirmed travel booking made before the introduction of the change, and who also hold a valid ETA, may still travel to the UK without a visa, only if their arrival is within the transition window. If your travel is booked for after 16 April 2026, you will need to obtain a visa before you leave St Lucia.

The change came into effect with very little notice. The St Lucian government says it was informed by letter just one day before the measures took effect. It is an unsettling development, but it is manageable, if you plan ahead.

What type of visa do I need?

If you are travelling to the UK for a brief stopover before connecting to India or another treatment destination, there are two options depending on whether you’ll pass through immigration or remain airside.

Direct Airside Transit Visa

If you stay in the international transit area of a UK airport without passing through UK immigration. This costs £70.

Standard Visitor Visa

If you do want to enter the UK, stay overnight, or spend one or more days in the country before your onward flight, a Standard Visitor visa costs £127 and covers stays of up to six months.

If you want to spend a day or two in London before a long-haul flight – whether to rest, connect with family, or simply take a break in the journey – the Standard Visitor visa is the best route.

The Home Office says the standard processing time for a visitor visa is around three weeks from the date of your biometric appointment. If you need a faster decision, a priority service is available for an additional £500, targeting a decision within five working days. Given how quickly circumstances can change when you are waiting on medical treatment, you may feel the priority service is worth the additional cost.

It’s important to note that if you’re travelling with family members, each person must make their own separate application and pay the fee individually. A family of four travelling together would pay visa costs of around £500 before any service charges are added.

How does this affect medical travel through the UK?

Many St Lucian patients travelling to India for specialist care fly via London – typically through Heathrow or Gatwick – because direct connections to South Asia from the Caribbean are limited. Some clients choose to use a UK stopover as an opportunity to see family members based in Britain, or simply to rest before a long onward flight.

This remains entirely possible. The visa requirement does not prevent travel through the UK – it simply means that proper documentation must be in place before you depart. The key difference from before is lead time. Where an ETA could previously be obtained online within hours, a Standard Visitor visa now requires a biometric appointment, supporting documents, and several weeks of processing time in standard cases.

This means that if UK transit or a UK stopover forms any part of your travel plan for medical treatment, you should factor in the visa application process from the very beginning – before flights are booked and certainly before a hospital admission date is confirmed.

What should you do now?

If you have travel booked before 16 April 2026

Check whether you hold a valid ETA. If you do, and your booking was confirmed before 5 March 2026, you should still be able to travel under the transition arrangements. Read your documentation carefully before you leave.

If your travel is planned for after 16 April 2026

Don’t assume your existing ETA is still valid – it’s not. Begin the Standard Visitor visa application process as early as possible. You can apply up to three months before your travel date. Do not leave this until your treatment date is imminent.

If you are at an early stage of planning your medical journey

If you are transiting airside only and will not enter the UK: You will still need to apply for a Direct Airside Transit Visa in advance of travel. This requirement is new and applies regardless of the length of your transit.
